To Complete Enrollment:
1.) Pay tuition

- At Central City Recreation Center (Checks made out to Central City Recreation Center) 615 South 300 East, Salt Lake City, 84111. (Hours M-F 9AM-6PM, Sat. 9AM-1PM). Central City’s Recreation phone number is 801-538-2062.
- Online (A $2 additional charge is added for online payments.)

Note: by registering for class you are stating that your dog is friendly with other dogs and people.
2.) Ask Your Vet to Fax Your Dog’s/Puppy’s Vaccination Records to Four Legged Scholars 888-643-0253. Your dog is required to have the following vaccinations: rabies, distemper, and parvovirus. Titer tests are accepted for adult dogs. Puppies must have at least their first set of vaccination (which does not include rabies until 14 weeks of age).
